Employee Stock Bonus Plan

Super D has set up an Employee Stock Bonus Plan for all of its employees that have a year of service and work 20+ hours a week. This plan is entirely employer sponsored. This plan also has January and July enrollments. There is a vesting schedule and employees are fully vested after 6 years in the plan.

Health Insurance

Super D pays 75% of the premium for employees only.

MEDICAL
Blue Cross of California: Blue Cross offers several different plans. You many choose from the HMO, PPO and Blue Card. The Blue Card is a program for employees that have a spouse or dependants in different states. The plan follows closely to the PPO. To find a doctor for any of the plans, go to www.bluecrossca.com and go under provider search. You can find either a doctor or if you want to be in a certain medical group you can search that way as well.

With any of the plans, Blue Cross provides coverage no matter where you are, even covers you when you are outside of the United States. The plans cover; Inpatient Service, Outpatient Medical Services, Ambulatory Surgical Center, Skilled Nursing Facility, Hospice Care, Home Health Care, Physician Medical Services, Short-Term Physical, Occupation or Speech Therapy, Chiropractic Care, Surgical Services, Health and Wellness Services, Emergency Care, Ambulance Services, Pregnancy and Maternity Care, Family Planning, Organ and Tissue Transplant, Mental or Nervous Disorders, Acute Alcoholism or Drug Abuse, and Smoking Cessation Programs.

Paid Time Off

  • YEARS 1-4 ACCRUE 8 HOURS OF PTO A MONTH
  • YEARS 5-10 ACCRUE 12 HOURS OF PTO A MONTH
  • YEARS 10+ ACCRUE 16 HOURS OF PTO A MONTH

Paid Time Off (PTO) may be taken as personal, sick or vacation times to allow you to rest, relax, and pursue special interests. PTO is for all full-time regular employees.

Dental

The plans that we offer include a DMO or a DPO. With the DMO, you must pick a dentist within the DMO network. The DMO has no deductible and no charge for a routine cleaning and x-rays. On most services, there is a co pay or none at all. With the DPO plan, there is an annual $50 deductible and then it is an 80/20 billing. There is also a maximum benefit of $1,000 for both plans. Vision

This vision plan is extremely easy to use and flexible. When you sign up for VSP (Vision Service Plan), you will receive the following:

  • Exam: Once every 12 months
  • Lenses: Once every 12 months
  • Frames: Once every 24 months
  • Contact Lenses Once every 12 months

VSP also gives discounted services on laser eye surgery. The co pay is $20 for the exam. To use this benefit, all you do is call an in-network doctor and give them your social security number, there is no need for an insurance card for this benefit. To find a doctor, go to www.vsp.com. This web site will also show you when you last used this benefit, where you went and what you had done.

401K

American Fund is the sponsor of our 401k plan. All eligible employees may participate after a year of service. The enrollment periods are January and July.

Birthday Gifts

All full-time employees will receive a paid day off for their birthday. This must be taken a week before or a week after the birthday. Each employee also received a certificate for one item up to a $20 value out of the warehouse.
